Pet Information Form and Boarding Contract

You will need:
•Boarding Contract and Pet Information Form
•Required vaccinations (rabies within 3 years, DHPP within 3 years, Bordetella within 1 year)
•Please also bring receipt or certificate with vaccine information from your Vet to keep on file at our office.
•Pets must be on a monthly flea preventative
•Your best friend's normal food, packaged in air tight container.
•Bedding, toys (limit 2); treats optional
•Your friends must be picked up by 9 AM to avoid being charged an additional day.
•Pets picked up on Sunday will be charged for a full day.
•Medication - if your friend is on a medication, consider putting in enough for an extra day. Please also place medications in a pill container for am and pm with identification of medication type. I dispense medication as a courtesy.
